QUESTIONS AND ANSWERS

WHO IS ELIGIBLE FOR COVERAGE?

You and your dependents, including your spouse and all unmarried children up to age 24 years of age are eligible for coverage.  A dependent's coverage will continue if they are incapable of self-sustaining employment by reason of any mental or physical handicap

and are chiefly dependent upon you as their guardian for support and/or maintenance.

ARE THERE ANY MEMBERSHIP OR MONTHLY ADMINISTRATION FEES?

NO, the only costs are your annual or semi-annual fees.

HOW MANY TIMES A YEAR CAN I VISIT DR. RAINEY?

You are encouraged to visit him on a regular basis.  With AMERICA'S PRE-PAID DENTAL PLAN, you're only limitations are as stated under BENEFITS.

CAN MY COVERAGE BE CANCELED?

Your coverage will remain in force as long as you pay your required

annual or semi-annual fees.

HOW DO I MAKE APPOINTMENTS?

Making an appointment is as easy as 1-2-3.  Simply call the office at 731-660-7767

at least 30 days after your Plan's effective date to schedule your first appointment.

HOW DO I PAY FOR SERVICES?

ALL PROCEDURES ARE PROVIDED AT A 30% DISCOUNT. 

The balance is to be paid directly to Dr. Rainey the day the service is provided.  

DOES THE PLAN COVER SERVICES PROVIDED BY A  SPECIALIST?

Services are ONLY covered when performed by Dr. Rob Rainey.

CAN FEES BE INCREASED?

AMERICA'S DENTAL ASSOCIATES reserves the right to change fees, 

but only when all plans of this form are increased.  AMERICA'S DENTAL ASSOCIATES will notify you within 30 days of our intent to increase fees.
